What Is Cluster Immunotherapy?

Cluster immunotherapy is an advanced allergy treatment designed to build the body’s tolerance to allergens more quickly than traditional methods. Instead of receiving a single injection per visit, patients undergo a series of shots in one session, typically lasting about two hours. This accelerated approach allows individuals to reach a maintenance dose in a shorter period, potentially reducing the overall treatment timeline. Cluster immunotherapy involves providing controlled doses of allergens at regular intervals, gradually desensitizing the immune system. This procedure is performed under professional supervision to ensure patient safety. Cluster immunotherapy should not be confused with regular immunotherapy (cancer treatment). Its primary purpose is to relieve allergic symptoms, not treat malignant cells.

Consultation & Preparation

Before starting cluster immunotherapy, a thorough consultation with our healthcare provider is essential to determine whether the procedure is appropriate based on the patient’s medical history and allergy test results. We may recommend specific pre-treatment medications, such as antihistamines and Singulair, to reduce the risk of adverse reactions during the injection sessions. Patients are also advised to avoid strenuous activities on the day of the procedure to minimize potential complications. Because cluster immunotherapy involves a concentrated exposure to allergens, careful preparation helps ensure both safety and effectiveness. Adhering to these medical guidelines is crucial for a smooth treatment experience.

Cluster Immunotherapy:
A Step-by-Step Procedure

Skin Preparation
Before each injection during cluster immunotherapy, the skin in the target area is carefully prepared to ensure cleanliness and reduce the risk of infection. Our physicians use an antiseptic solution to remove dirt, oils, or bacteria. This step helps create an optimal environment for the injections, ensuring that the allergens are administered accurately and safely. Proper skin preparation is an essential part of the procedure to maintain hygiene and minimize any potential complications.
Patients receive 2-3 allergy injections at scheduled intervals, usually every 20-30 minutes. The shots contain small, controlled doses of allergens to help the immune system gradually build tolerance. We closely supervise the process to assess the individual’s response and address any immediate reactions.
Because cluster immunotherapy involves a higher concentration of allergens in a shorter period, our doctors observe patients for some time after the final injection. This monitoring phase allows us to promptly manage adverse reactions, such as swelling, redness, or more severe allergic responses. Proper medications and support personnel are readily available to ensure patient safety.
A typical cluster immunotherapy session lasts about two hours, allowing people to receive intense exposure to allergens in one visit. Patients may be given additional instructions for at-home care and follow-up appointments to continue their treatment plan.
After completing the build-up phase, we transition to the maintenance stage. This period typically lasts about 5 years, during which time the body’s immune system continues to develop, increasing its ability to tolerate exposure without triggering an allergic reaction by regular controlled exposure to allergens.

What Happens After Cluster Immunotherapy Sessions

After cluster immunotherapy, patients should follow specific guidelines to ensure a smooth recovery. Some individuals experience mild soreness, swelling, or redness at the injection sites. These effects usually subside within a few hours. If any discomfort persists, we may recommend an over-the-counter pain reliever.
To reduce the risk of irritation or further reactions, patients should avoid vigorous exercise or activities that might cause excessive sweating for at least 24 hours after the procedure. They must also monitor for unusual symptoms, such as difficulty breathing, hives, or dizziness, and seek medical attention if these side effects arise. Maintaining the prescribed medication regimen, including antihistamines or other drugs as directed, can help manage mild reactions and support the body’s adjustment to the allergens. Regular follow-ups are crucial to track progress and adjust the treatment plan.

Candidates for Cluster Immunotherapy

Individuals With Severe Allergies
Cluster immunotherapy is typically considered for patients who suffer from severe allergies to substances like pollen, dust mites, mold, or pet dander. These individuals may find that traditional treatments, such as antihistamines or nasal sprays, do not provide sufficient relief. Cluster immunotherapy offers an accelerated approach for patients with these conditions to help build tolerance to their specific allergens.
Our treatment is generally suitable for adults and children, provided they are at least 5 years old. We carefully evaluate each individual to determine whether their allergy is sufficiently severe to warrant cluster immunotherapy. Most patients benefit from the procedure, especially if their symptoms are persistent and bothersome.
Candidates for cluster immunotherapy often include people who have not had significant relief from other allergy treatments, including medications or nasal sprays. Our procedure may offer substantial improvements when traditional therapies are ineffective. Many individuals require more aggressive measures to manage their allergic reactions.
Candidates for cluster immunotherapy should not have certain conditions, such as uncontrolled asthma or other severe diseases, that could make the treatment riskier. Our physicians carefully evaluate each patient’s medical history to ensure they can tolerate the injections and allergens. Those with chronic health issues or diseases that compromise the immune system may not be eligible for this procedure.

Benefits of Cluster Immunotherapy

Faster Allergy Relief

One of the primary benefits of cluster immunotherapy is its ability to get faster results than traditional allergy treatments. By receiving multiple injections in a single visit, patients can achieve the benefit that it typically takes a month of conventional allergy injections to get in just a few hours. This accelerated approach allows individuals to build tolerance more quickly.
Cluster immunotherapy offers greater efficiency by condensing the treatment process into fewer sessions. Rather than receiving weekly or bi-weekly injections over an extended period, patients can complete the buildup phase in a fraction of the time. This more intensive approach allows for a quicker transition to the maintenance phase.
Patients who undergo cluster immunotherapy often experience long-term relief from allergy symptoms. The rapid buildup of tolerance to allergens can lead to sustained improvements, with most patients requiring far fewer immunotherapy session visits before the maintenance phase is reached. Over time, the procedure may decrease the need for other allergy medications, providing a more natural and long-lasting solution.
Cluster immunotherapy offers significant convenience for busy individuals. By consolidating multiple injections into one visit, patients can reduce the number of office visits required throughout treatment. This approach helps them manage therapy without frequent, time-consuming appointments.

First and foremost let me start by saying that these people did one hell of a job figuring out my sinus problems. When I first arrived there, I explained what was going on with me having excessive inflammation in the sinus…after they tested me, I found out that I was being exposed to dust mites which gave me a strong reaction. I followed the doctors guides lines in terms of cleaning my room and ever since, my sinus inflammation went away for good and now I have a healthy sinus. Thank you for everything.

Dr. Engler is great and his staff, both medical and administrative, are wonderful. Dr. Engler is competent, caring, and listens better than any doctor I’ve ever had, constantly and patiently asking if I have any more questions. The staff is great. The nurses are very friendly and helpful. The desk staff is the same. I have been going here over a year and have always received caring and competent treatment. I highly recommend them.

Hands down the best place to go for Allergies and Asthma. The staff is incredibly kind and caring and so professional and helpful. I’ve been going here for over 30 years and they have gone out of their way countless times to make sure that I was well-cared-for. The nurses are excellent and always make sure to ask how my family is doing not to mention that they give the most painless allergy shots around. They remember little details about me and always make sure to give me a lollipop for my son, even when he’s not with me. Brooke is a great diagnostician and listener and each time I have an appointment with her, I always leave feeling better. The same is true for Dr. Engler.

My 10 year old son also has allergies and asthma and the staff makes sure that he feels well taken care of and comfortable, despite his having Autism and ADHD. He actually looks forward to going there with me, whether it be for my allergy shots or even when he needed allergy skin testing. When you’re a patient at the Allergy and Asthma clinic, you are treated as if you are the only patient they have, even when the waiting room is full. Dr. Engler has hired truly excellent staff, from the front desk people to the nurses and medical assistants. If you have allergies and/or asthma and you live on the Peninsula or even in the city, I highly recommend that you visit the Allergy and Asthma Clinic. They are truly amazing and will make you feel so much better.

I had an amazing experience at this clinic. Dr. Engler and the staff were all extremely welcoming, warm, and thorough in their various lanes of work from the front desk to billing to the assistants to Dr. Engler himself.
One thing I wish I knew before coming in was that the food test covers 80 allergens, and the environmental test covers 80 allergens meaning if you want to do both you need to come two separate days. This is kind of a hassle but definitely worth it since you’ll be tested for 160 allergens whereas most clinics only test 80 total.
Through the tests I figured out what I was allergic to and was advised to use an over the counter medication which has drastically helped and eliminated the symptoms I was experiencing. Dr. Engler also covered all the things I needed to do to reduce my symptoms via my lifestyle which was super helpful with my symptom reduction as well. I’d highly recommend this clinic! Insurance covered the tests and visits. (I have anthem blue cross)

I’ve been a patient at The Allergy and Asthma Clinic for over 12 years, and I can’t speak highly enough of the care I’ve received. Growing up with severe pollen allergies, I’ve been on and off allergy shots for years, and this clinic has truly been a lifesaver for me.
From the moment you walk in, you’re treated like family. The staff is incredibly kind, caring, and genuinely invested in your well-being. I have never had a negative experience with anyone here; everyone is consistently friendly and supportive.
Dr. Engler is exceptional! He has a remarkable ability to quickly and effectively address any situation. His expertise and compassionate approach have made a significant difference in managing my allergies.
If you’re struggling with allergies or asthma, this is the place to go. I wholeheartedly recommend The Allergy and Asthma Clinic to anyone seeking top-notch care.
